ABSTRACT

Abstract@#Allergic diseases can occur in all systems of the body, covering the whole life cycle, from children to adults and to old age, can be lifelong onset and even fatal in severe cases. Children account for the largest proportion of the victims of allergic disease, Children s allergies start from scratch, ranging from mild to severe, from less to more, from single to multiple systems and systemic performance, so the prevention and treatment of allergic diseases in children is of great importance, which can not only prevent high risk allergic conditions from developing into allergic diseases, but also further block the process of allergy. At present, there is no consensus on the management system of allergic children in kindergartens and primary schools. The "Consensus on Allergy Management and Prevention in Kindergartens and Primary Schools", which includes the organizational structure, system construction and management of allergic children, provides evidence informed recommendations for the long term comprehensive management of allergic children in kindergartens and primary schools, and provides a basis for the establishment of the prevention system for allergic children.

ABSTRACT

Objective:To evaluate delayed laparoscopic cholecystectomy (DLC) after percutaneous transhepatic gallbladder drainage (PTGBD) in acute cholecystitis.Methods:Clinical data of 64 patients who were diagnosed moderate (grade Ⅱ) acute cholecystitis by the 2018 Tokyo Guidelines in acute phase and underwent delayed LC at our hospital from Jan 2018 to Jan 2021 were compared between two groups ie PTGBD treatment (21 cases)in acute stage before DLC and DLC without PTGBD group (43 cases). The difficulty score of TG18 was used to evaluated every surgical procedure of the cases by reviewing the operation videos.Results:Patients in DLC after PTGBD group had a longer hospital stay and operation time, more blood lose and higher difficulty score than the DLC without PTGBD group(all P<0.05). There was no statistically significant difference in the conversion rate and morbidity rate between the two groups( P>0.05). Conclusion:This study fails to show there is any if ever benefit of PTGBD before DLC over DLC without PTGBD in the management of Grade Ⅱ acute cholecystitis.

ABSTRACT

ABSTRACT Objective: Recently, several studies have found that obesity had a protective effect against varicocele, but no meta-analysis has confirmed this finding. Therefore, we conducted this meta-analysis to investigate the association between body mass index (BMI) and varicocele. Material and Methods: We searched for studies in PubMed, Science Direct and the Cochrane Library from inception until February 2018. The association between BMI and varicocele was assessed by pooling the odds ratios (ORs). Results: Eleven eligible studies with a total study population of 1.376.658 participants were included in our analysis. According to BMI, the subjects were defined as belonging to the obese, overweight and underweight groups. Our results showed that the obese group had a lower risk of varicocele when compared with the normal weight group (odds ratio [OR] 0.46, 95% confidence intervals [CIs] 0.37-0.58). Additionally, an overweight BMI had a protective effect against varicocele (OR 0.70, 95% CIs, 0.56-0.86). However, underweight patients had a more than 30% higher risk of varicocele (OR 1.31, 95% CI, 1.04-1.64). Furthermore, there was no publication bias in any of the analyses. Conclusions: Our study demonstrates that BMI is negatively associated with the presence of varicocele.

ABSTRACT

Objective To investigate the current prevalence of human soil-borne nematode infections in Yunnan province, so as to provide the scientific evidence for formulating the soil-borne nematodiasis control strategy in the province. Methods In 2015, a total of 20 survey sites were sampled in 10 counties (cities) of Yunnan Province using the stratified cluster random sampling method. Stool samples were collected from all local permanent residents at ages of one year and older in each survey site, and the soil-borne nematode eggs were identified using the modified Kato-Katz technique and the egg number was counted. In addition, the hookworm species was identified using the filter-paperculture method, and Enterobius vermicularis eggs were detected using the cellophane tape method in children at ages of 3 to 6 years. Results A total of 5 067 residents received stool examinations, and 950 residents were detected with soil-borne nematode infections, with an overall prevalence rate of 18.75%. The prevalence of Ascaris lumbricoides, Trichuris trichura and hookworm was 7.52%, 8.47% and 9.02%, respectively. Among 446 children detected using the cellophane tape method, 5 children were detected with E. vermicularis infections. Among the 160 residents with hookworm infections, there were 139 residents with Necator americanus infections (86.88%), 16 with A. duodenale infections (10.00%) and 5 with mixed infections (3.12%). Mild A. lumbricoides (67.98%, 259/381), T. trichura (88.58%, 380/429) and hookworm infections (94.53%, 432/457) were predominant. Among the four ecological zones, the highest prevalence of human soilborne nematode infections was found in the East Tibet-South Sichuan Ecological Zone (31.79%), and among the 10 survey counties (cities), the greatest prevalence was seen in Gongshan Derung and Nu Autonomous County (50.13%), while the lowest prevalence was found in Ninglang Yi Autonomous County (0.40%). The prevalence of human soil-borne nematode infections was 5.67% (43/759), 26.67% (610/2 287) and 14.70% (297/2 021) in high-, moderate- and low-economic-level regions, respectively. There were no significant differences in the prevalence of human soil-borne nematode infections in terms of ecological regions, survey counties (cities) or economic development levels (χ2 = 342.20, 814.60 and 201.34, all P < 0.05). There was no significantdifference in the prevalence of human soil-borne nematode infections between male (18.21%, 441/2 422) and female residents (19.24%, 509/2 645) (χ2 = 0.89, P > 0.05), and soil-borne nematode infections were detected in residents at all age groups, with the greatest prevalence found in residents at ages of 1 to 9 years (25.88%). In addition, the highest prevalence of soil-borne nematode infections was seen in residents with the Dulong Ethnic Minority (82.09%), in preschool children (25.06%) and in illiterate residents (24.80%), and there was no age-, ethnicity-, occupation- or education level-specific prevalence of soil-borne nematode infections detected (χ2 = 46.50, 1 016.96, 36.33 and 52.43, all P < 0.05). Conclusions The prevalence of human soil-borne nematode infections remains high in Yunnan Province. The management of soil-borne nematodiasis requires to be reinforced among low-age children, farmers, old people and residents with low educations levels or ethnic groups.

ABSTRACT

Diabetes mellitus is a metabolic disease characterized by chronic hyperglycemia. Recently, the number of diabetic patients in the world is rapidly growing, which has brought a heavy burden to the society. At the same time, changes in lifestyle lead to a general decline in the quality of sleep in the population. Epidemiological and clinical studies have shown a close relation between the development of diabetes and sleep disorders, which may form a vicious circle. Sleep disorders promote the occurrence of type 2 diabetes and affect the glycemic control of diabetic patients; on the other hand, diabetic patients, especially in poor glycemic control, often have sleep disorders. This review discusses the relationship between type 2 diabetes and sleep disorders based on related research, and may provide new strategies for future treatment of diabetes.

ABSTRACT

·AIM: To observe the clinical effects of microincision vitrectomy combined with internal limiting membrane (ILM) peeling for high myopia patient with macular hole retinal detachment (MHRD). ·METHODS: This was a retrospective non-randomized controlled clinical study. A total of 26 eyes of 26 patients with high myopic MHRD from January 2011 to December 2016 were included. All eyes underwent 23G pars plana microincision vitrectomy combined with ILM peeling. The preoperative and postoperative best corrected visual acuity ( BCVA ), intraocular pressure, ocular anterior segment and fundus examination were observed, and the anatomical closure of macular hole was checked by optical coherence tomography ( OCT). The relationships between final BCVA and these parameters ( age, GASS stage, onset time, OCT pattern of MH closure, initial vision) were examined by regression analysis. ·RESULTS: The postoperative MH closure rate of high myopia MHRD was 58% . OCT images of the repaired MH in high myopia were categorized into 3 patterns: U-type (3 eyes) with relative normal foveal contour; V-type (4 eyes) with steep foveal contour; W-type (8 eyes) with foveal defect od neruosensory retina, but without warped hem of retinal hole or cystic formation. Multivariate Logistic regression analysis showed that postoperative BCVA was correlated with the OCT patternts of closed MH and initial vision ( P< 0. 05 ). The postoperative visual acuity of U-type closed MH was 6. 9 times higher than that of W-type. ·CONCLUSION: Microincision vitrectomy combined with ILM peeling is a safe and effective surgical treatment for high myopia patient with macular hole retinal detachment. The postoperative visual acuity was correlated with the OCT patterns of closed MH and initial vision.

Brain magnetic resonance imaging (MRI) of the elderly often reveals white matter changes (WMCs) with substantial variability across individuals.Our study was designed to explore MRI features and site-specific factors of ischemic WMCs.Clinical data of consecutive patients diagnosed with ischemic cerebral vascular disease who had undergone brain MRI were collected and analyzed.Multi-logistic regression analysis comparing patients with mild versus severe WMCs was performed to detect independent associations.Analyses of variance (ANOVAs) were used to detect regionally specific differences in lesions.We found that lesion distribution differed significantly across five cerebral areas,with lesions being predominant in the frontal lobe and parieto-occipital area.To explore WMCs risk factors,after adjusting for gender,diabetes mellitus,and hypertension,only age (P<0.01),creatinine (P=0.01),alkaline phosphatase (ALP) (P=0.01) and low-density lipoprotein cholesterol (LDL-C) (P=0.03) were found to be independently associated with severe WMCs.Age (P<0.001) was strongly associated with WMCs in the frontal lobe while hypertension was independently related to lesions in the basal ganglia (P=0.048) or infratentorial area (P=0.016).In conclusion,MRI of WMCs showed that ischemic WMCs occurred mostly in the frontal lobe and parieto-occipital area.The infratentorial area was least affected by WMCs.Typically,age-related WMCs were observed in the frontal lobes,while hypertension-related WMCs tended to occur in the basal ganglia and infratentorial area.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the feasibility and the attention of perioperative management of goat lumbar fusion model for individualized 3D printing technology.</p><p><b>METHODS</b>According to preoperative X-ray and CT three dimensional reconstruction data of 10 males Boer goat's lumbar(1-2 years old, weight 35-45 kg), the preoperative open height were determined, meanwhile, according to the theoretical entry point of nails, the length of steel plate, arc, and setting position, screw length for reference were determined, the lumbar lateral anterior plate was designed and 3D-printed. Goats lied on the right side, under the general anesthesia, the lumbar vertebrae of the goats and the adjacent intervertebral disc were resected, and the titanium cage after the bone graft was implanted into the goat, the 3D-print lateral bone plate was fixed. After operation, feeding, fluid infusion, anti infection, postoperative complications management, respiratory digestion perioperative management were performed.</p><p><b>RESULTS</b>The 10 models for goats were successful in results. Postoperative X-ray film and three-dimensional reconstruction of CT showed that titanium cage and bone plate were in good position and reliable. Three months after the operation, CT 3D reconstruction and micro-CT of the goat were observed, and the fusion of the spine was observed. Imaging studies showed that the fusion of the lateral bone plate fixation titanium cage was both at the end of the titanium cage and the dense bone trabecular formation between the vertebral bodies.</p><p><b>CONCLUSIONS</b>The 3D printing technology sets up the goat lumbar spinal fusion model successfully, which is a kind of effective, more successful, reliable and stable method, perioperative management. The method is scientific, practical, and more humanized, to ensure that lumbar lateral successfully implanted the nail plate of lateralanterior internal fixation system, with reduction of occurrence of surgical complications.</p>

OBJECTIVE@#To study and measure the anatomic structure of lumbar vertebral endplate structure in healthy adults by computed tomography(CT) technique in order to provide a useful guidance for the optimal design and clinical application of lumbar prostheses.@*METHODS@#Sixty healthy adults (male and female equals) were recruited for full-waist CT scan after signing the informed consent form in the imaging department of the Second Affiliated Hospital of Xi'an Jiaotong University. The scanning data was imported into the computer aided software Mimics 16.0 for 3D reconstruction and measurement. The acquisition indexes included median sagittal diameter, maximum coronal diameter, concavity depth, median sagittal depression angle, coronal depression angle and so on. Finally, the collected data were statistically analyzed by the statistical software.@*RESULTS@#The median sagittal diameter and the maximum coronal diameter of the upper and lower endplates were not only different between the different sexes(0.05), but had a little change from L₁ to L₅, fluctuating from 1.5 to 2.0 mm and from 2.2 to 3.9 mm, respectively. In the same sequence, the concavity depth of lower endplate in males was greater than that of upper endplate, and the difference was statistically significant(0.05). Sagittal concavity angle and coronal concavity angle of upper and lower endplates changed slightly with the increase of vertebral order, and there was no gender difference in sagittal and coronal concavity angle of most vertebral sequences (>0.05). Statistics showed that the largest concavity near the caudal lumbar endplate was located on the dorsal side of the endplate plane.@*CONCLUSIONS@#The anatomical structure of the lumbar endplate is very complicated. It is important to master the anatomical parameters of the endplate and make full use of CT before operation for the development and clinical application of the lumbar prosthesis.

ABSTRACT

@#AIM: To discuss the treatment and to evaluate the therapeutic effect by reviewing a group of postvitrectomy diabetic vitreous haemorrhage(PDVH)cases after vitrectomy for proliferative diabetic retinopathy(PDR). <p>METHODS: Retrospective analysis of 12 cases of 12 PDR patients with PDVH in our hospital from September 2015 to June 2017. First, conservative treatment was performed and then surgical treatments were performed on patients with large amount of bleeding and poor effect of conservative treatment(including anterior chamber flushing, vitreous lavage, vitrectomy, or combination of cataract surgery). The patients were observed and analyzed during and after surgeries. <p>RESULTS: The mean duration between the first surgery and PDVH ranged from 1d to 10mo(mean 61.58±92.69d). The cases of early and late PDVH was 8 eyes and 4 eyes, respectively. Two eyes were treated with conservative treatment and the bleeding were absorbed after 2wk-1mo. Ten eyes without obvious improvement after treatment or intraocular pressure(IOP)increased again received surgical treatment, in which 2 eyes were treated with simple anterior chamber flushing, 8 eyes underwent vitreous lavage(2 eyes combined with cataract surgery, 1 eyes combined with silicone oil injection). Seven eyes received supplementary photocoagulation. Till the last follow-up, all vitreous haemorrhages resolved, and 9 eyes had better visual acuity. <p>CONCLUSION: PDVH can be induced by various reasons and appropriate measures should be taken by regulating blood glucose, improving operative skills and timely symptomatic treatments to improve the visual acuity.

Bacterial resistance has become a serious problem that the whole world is facing.Routine bacterial drug resistance testing methods such as K-B test and dilution method are time-consuming and incapable to identify the specific drug resistance mechanism , which can not meet the needs of clinicians. MALDI-TOF MS overcomes the shortcomings of traditional methods and has broad application prospects in detection of bacterial resistance and drug resistance mechanism.This article reviews the recent progress on the application of MALDI-TOF MS in detection of bacterial drug resistance mechanisms.

ABSTRACT

Upper cervical fracture-dislocations are series of pathological injuries occurred in occipital, atlas and axis caused by traumatic impact, which often results in severe clinical consequences, such as paraplegia, quadriplegia, and even death. In light of the potential severe clinical outcomes, it is important to define the type of upper cervical fracture-dislocations depending on which the clinician can deal with the disease properly. Owing to its specific anatomical structures, the upper cervical fractures occurr to the unique osseous structures or ligamentous connections always present in predictable patterns. Common fracture include occipital condylar fracture, atlanto-occipital dislocation, atlas fracture, and the three types of fractures of axis, including odontoid fracture, Hangman's fracture and axis body fracture. In this paper we will review the literatures about the classification and management of upper cervical fracture, to help the clinicians to treat their patients better.

@#AIM: To summarize the eye infection distribution of pathogens and drug resistance characteristics in our hospital in latest 5a, hoping to provide a theoretical basis for clinical anti-infective therapy and nosocomial infection control. <p>METHODS:In this study, we collected positive strains isolated from the hospitalized patients in Renmin Hospital of Wuhan University from January 2011 to December 2015. Data were analyzed with WHONET 5.6 software. <p>RESULTS: A total of 4 486 cases of ophthalmic inpatients were included in the study, 736 strains of pathogenic microorganisms were isolated, including 510 gram-positive bacteria, 107 gram-negative bacteria and 119 fungi. The most common type of gram positive bacteria as the epidermis staphylococcus(247 strains), followed by coryne bacterium(153 strains). The most common type of gram negative bacteria for pseudomonas aeruginosa(39 strains). The most common type of fungi for sickle bacterium(77 strains), followed by aspergillus strain(31 strains). The staphylococcus was sensitive to rifampicin, trimethoprim, linezolid, vancomycin and teicoplanin. The Streptococcus pneumoniae was sensitive to levofloxacin, linezolid, vancomycin and teicoplanin. The drug sensitive rates of non-fermenters to cefoperazone/sulbactam and colistin, enterobacteriaceae to imipenem, meropenem and levofloxacin were all 100%. The detection rates of methicillin-resistant staphylococci and penicillin-resistant Streptococcus pneumoniae were 37.5% and 40.0%, respectively. <p>CONCLUSION:We should strengthen the monitoring of pathogen resistance, to keep abreast of the epidemic characteristics of ophthalmic infection pathogens and drug resistance trends, which are of great significance for the rational application of antibiotics in clinical anti-infective treatment, reducing the emergence of drug-resistant bacteria and hospital infection control measures.

ABSTRACT

? AIM: To explore the curative effect of dacryocystorhinostomy under endoscopy with mitomycin for the treatment of chronic dacryocystitis.?METHODS: Totally 73 cases ( 78 eyes ) with chronic dacryocystitis were treated with dacryocystorhinostomy under endoscopy with mitomycin and followed up for 6-12mo.?RESULTS: In the 73 patients, 66 cases with 70 eyes (90%) were cured, 2 cases with 3 eyes (4%) improved, 5 cases with 5 eyes ( 6%) not changed. In the recurrent 5 eyes, 2 eyes were treated under endoscopy to remove granulation, enlarge the opening, then anesthetic tube was placed after cotton sheet with 0. 4g/L mitomycin was put on the incision for 5min. The rest 3 eyes were treated in superior hospital with laser, and all were successful. There was no severe complication observed.?CONCLUSION:Dacryocystorhinostomy under endoscopy with mitomycin for chronic dacryocystitis is effective.

OBJECTIVE: To establish a methodology to dilute blood sample with compound matrix modifier,and to determine the blood level of nickel using improved graphite furnace atomic absorption spectrometry( GFAAS). METHODS: The blood samples( 100. 00 μL) were diluted 5 times in 0. 12 g / L mass concentration of palladium chloride combined with 1. 00 %(V /V) Triton X-100 and 0. 10%(V /V) nitric acid,and the blood level of nickel was detected by GFAAS. RESULTS: The good linearity range of nickel mass concentration in blood was 0. 30-25. 00 μg / L,the correlation coefficient was 0. 999 7,the detection limit was 0. 30 μg / L,the minimum detectable mass concentration was 1. 50 μg / L,the recovery rate was99. 00 %-102. 75 %. The within-run and between-run relative standard deviations were 1. 43 %-3. 70 % and 2. 20 %-5. 03 %,respectively. Blood samples can be stored for at least 7 days at 4 ℃. CONCLUSION: This method is simple for operation,with high accuracy and precision,which is suitable for determination of nickel in blood in normal individuals and workers with occupational exposed to nickel.

Objective To investigate the tendency of distribution and drug-resistance of the causative organisms of urinary tract infections(UTIs)in Wuhan,and provide reliable evidence for clinical treatment.Methods Analyzed the 5 378 stains of pathogen isolated from the urine of patients in hospital.The bacteria isolates were identified with BD Phoenix-100 while can-dida isolates were identified by color plate.Results A total of 5 378 stains of pathogen had been isolated.There were 2 945 stains (54.8%)of Gram-negative bacteria,1 657 stains (30.8%)of Gram-positive bacteria,776 stains (14.4%)of fungus. The rates of Escherichiacoli resistant to penicillin were highest (>83%),and there were no carbapenem-resistant strains. There were vancomycin and linezolid-resistant Enterococcispp strains,the lowest dection rates of which were 0.3%.The de-tection rate of MRCNS was over 83%.Conclusion Escherichiacoli was the most common pathogens of urinary tract infec-tion,and theβ-lactamase inhibitor complex can be used as empirical treatment of E.coli infections.Thedetection rate of MRCNS increased,which shoud be kept a watchful eye on.

ABSTRACT

To investigate the utility of optomap panoramic 200Tx in screening fundus disease among the patients after cataract surgery. ●METHODS: From November 18 th to December 31st , 2013 all 146 post- cataract surgery patients were recruited. All non - mydriatic fundus images were taken with the optomap panoramic 200Tx and were diagnosed by one masked retinal specialist. Non - mydriatic direct ophthalmoscope exam and mydriatic slit-lamp lens exam were also done by other two masked specialists. Comparisons of the three methods were made. ●RESULTS: Among 146 patients ( 161 eyes), 40 eyes (24. 8%) of retinal lesions was detected by non-mydriatic direct ophthalmoscope exam, 59 ( 36. 7%) by non -mydriatic optomap 200Tx exam, and 61 (37. 9%) by slit-lamp lens exam. Nine eyes ( 5. 6%) needed medical intervention immediately. Results of optomap 200Tx exams and slit - lamp lens exams were similar without statistically significant difference ( P > 0. 05), better than direct ophthalmoscope exam ( P ● CONCLUSlON: Opacification of the refractive medium makes thorough fundus examination impossible. So post-operative fundus examination is highly necessary and should be a routine. Optomap panoramic 200Tx, which shows no statistically difference from mydriatc slit- lamp lens exam, is a convenient and feasible method in discovering fundus pathological changes.

ABSTRACT

OBJECTIVE To investigate antibiotic resistance in nosocomial infections with Acinetobacter baumannii from lower respiratory tract in the intensive care unit(ICU) and guide clinically rational use of drug.METHODS Antibiotics susceptibility tests in 109 clinical isolates of A.baumannii from lower respiratiory tract were performed by K-B disk diffusion method.The results were judged according to CLSI 2007.As controls,standard strain was used simultaneously.RESULTS The sensitivity rate to cefoperazone/sulbactam was the highest,arriving at 77.1% and followed by imipenem(57.8%) and meropenem(54.9%).The other antibiotics were below 50.0%,generally.The serious cross-resistance phenomenon was in present.Pan-resistant strains had been detected.CONCLUSIONS The drug-resistant status in nosocomial infections with A.baumannii from lower respiratory tract in ICU is very serious.We should continue to strengthen the monitoring of the antibiotic resistance and prevent the outbreak epidemics of drug resistant strains in ICU.
